PROFESSIONAL STUDENT EXCHANGE PROGRAM (PSEP)

You must meet the following requirements to apply for Arizona WICHE Certification:

            1.  be able to provide proof of United States citizenship;

            2.   be able to provide proof of residency of the State of Arizona for a minimum of five years prior to the commencement of training;

            3.   be able to provide proof that you have maintained at least average (2.5) grades while completing your pre-professional studies.

If you meet all three criteria, follow the directions listed below, beginning with reading the brochure, which describes the program and explains in detail the requirements placed on those who are funded through the program.

DIRECTIONS:

1. Read carefully the Arizona-Professional Student Exchange Program brochure (PDF) AND the New Requirements for Verification of U.S. Citizenship(PDF).

2. Print application and type or print plainly all requested information.  Incomplete applications will be returned.

3. By October 15th of the year preceding your professional training, return to:

4. Forms required:

   •  All fields of study

     • Application for WICHE Certification (must be printed due to requirement for notarization)

     • Copy of Verification of U.S. Citizenship

     • Consent and Waiver Form (2 copies)

   • Dental applicants only

     • Statement of Understanding for WICHE Dental Applicants (1 copy)

   • Veterinary Medicine applicants only

     • Statement of Understanding for WICHE Veterinary Medicine Applicants (2 copies)

     • Statement of Intent for WICHE Veterinary Medicine Applicants (2 copies)

     • Cooperative Admissions Procedures

 Note: Contracts will be provided to those who are supported.
